Physicals and Rank One Online Forms


All students participating in the band program must be cleared in Rank One prior to doing any outside activity.  To be cleared in Rank One, every student must:

  1. Complete a physical. If you did not participate in the Physical Day at the Fall Preview on May 17th, you can do this at your family physician, urgent care, or minute clinic. The official Humble ISD physical must be filled out and stamped by your health care professional. Physical forms can be found and printed by clicking here. Physical forms can be submitted two ways:
      • Upload them online when you complete your Rank One Forms (link in step two) OR

          • Submit them in person to the Athletic Department at the Humble ISD Administration Building (located behind Hobby Lobby neat Dearbrook Mall) between 10am-2pm on June 11, June 25, July 9, July 16, or July 23
    1. Fill out the required online forms in Rank One. Click here and follow the directions for complete the forms. There should be an Emergency contact form, a UIL signature page, and the Humble Sportsmanship Form.
          • IMPORTANT: Sign in as a guest. If you have already turned in a physical, SKIP the Medical History/Physical Upload. If you have not turned in your physical, you can upload it here. 

     

    Students will NOT be able to participate in ANY outdoor marching band related activity until BOTH of these things are completed. Please do these two things ASAP so that there is time for the trainers to clear your paperwork and address any issues that may arise. Failure to turn these in early will delay your participation with the marching band until paperwork is clear.
